Santa Has Gynecomastia
It’s that time of
year—visits with friends and family; the fragrance of turkey roasting in the
oven; the familiar traditions and memories that all of us enjoy so much! Because
it’s “the season,” it seems appropriate that we share a frank discussion about gynecomastia,
because we suspect that Santa has it. Certainly, Dr. Kavali and her staff at
Kavali Plastic Surgery in Atlanta are
more likely to notice these kinds of things.
Since Santa is known to be so fond of cookies and milk, leading to obesity (sorry, but it’s true), we’re pretty certain he has gynecomastia, as most obese men have it. Although we aren’t sure Santa would be interested in eliminating his man-boobs or—the more proper description—enlarged breast tissue, we do know that lots of men in Atlanta suffer the embarrassment of gynecomastia. Since there’s only one Santa, we can safely assume that the other men with enlarged breasts (A) aren’t Santa and (B) might like to know what can be done to safely and effectively treat this concern.

Gynecomastia is surprisingly common—estimates are that at any given time, more than half of all men have enlarged breasts! There are two times in a male’s life when enlarged breasts tend to occur—during puberty and during a man’s later years. Why is this the case? Because hormonal swings, common during puberty and as a man ages, can cause the breasts to enlarge. Doubtless, many of you have noted the slightly puffy breast tissue in many male infants—it’s caused by reserves of estrogen from the mother. When that estrogen reserve begins to dissipate between 4-6 weeks of age, the male baby’s breasts become more normal in size. It’s the magic of hormones!
There are other reasons for enlarged breasts in men, however. Certain chemicals and medications can cause gynecomastia, particularly steroids and marijuana. There are also medical problems that can cause gynecomastia. Therefore, enlarged male breasts, although usually of no particular medical concern, should be checked out by a family physician or an endocrinologist before plans are made for any intervention by a cosmetic surgeon.
Although enlarged breast tissue in men is usually not a medical concern, it can cause significant emotional distress. It’s a rare teenaged boy or adult male with gynecomastia that isn’t terribly embarrassed by it. Boys who previously enjoyed swimming and wore t-shirts and tank tops suddenly hate the beach, refuse to enjoy the pool with the family and may even become withdrawn, shying away from boy-girl interactions at exactly the age when they’re learning how to relate to the opposite sex. Worse, they can bear the brunt of teasing from other boys when changing clothes after gym or sporting events. Obviously, this embarrassment isn’t limited to teenaged boys. Grown men with gynecomastia also avoid situations in which exposure of their enlarged breasts might happen—the aforementioned beach as well as the gym.
Happily, there are several options to help men with gynecomastia. One is liposuction. Unless the skin is stretched to the point where sagging is likely to occur after removal of the excess fatty tissue, liposuction is usually adequate. However, for men with excessively large breasts and/or skin sagging, excising some of the skin surrounding the breasts may be necessary along with removal of the fatty tissue.
In general, male breast reduction is a simple out-patient procedure that takes only a couple of hours. Recovery is rapid; most men and teen boys need only a couple days from work or school following a breast reduction procedure. It’s quite the rewarding treatment once recovery is complete and the embarrassing problem has been resolved.

Got Cellulite?
Oh, the dreaded lumps! The cottage cheese dimples! And the complete unfairness that 80% of women—even thin women—get cellulite, yet men rarely do! Why?
What Is Cellulite?
On the surface of the skin, cellulite looks like dimples and bumps. In places where the skin’s connective tissues are stretched to their limits by fat cells beneath the skin, cellulite appears. The "dimples" occur at the points where the surface of the skin connects to the tissues beneath, and the "bumps" are where the fat cells are pushing outward on the skin.
What Causes Cellulite?
A genetic predisposition, hormone levels, hydration, diet, exercise, stress, habits, and some medications can all contribute to the development of cellulite. Estrogen will cause the fat cells to clump together, for unknown reasons.
Why Don’t Men Get Cellulite?
Actually, men do get cellulite. You just can’t see it as well as you can on a woman! Cellulite is more visible on a woman’s body than on a man’s body because men’s body fat is concentrated inside the belly and abdomen, while women’s body fat is located nearer the skin. Further, women’s skin is thinner, making the cellulite more visible. And, last, women carry more fat than men.
Does Anything Eliminate Cellulite?
There are many cellulite reduction creams on the market today that claim to eliminate cellulite, but the truth is that cellulite is much too deep beneath your skin’s surface to be eliminated by mere creams. Cellulite removal requires more than a cream!
Unfortunately, that dimpled skin on your thighs, hips, and buttocks is very resistant to diet and exercise, although avoiding exercise and prolonged sitting (like at the office) can certainly increase your propensity to develop cellulite.

Do You Need to Change Your Breast Implants?
I’d like to talk to you about the “shelf life” of breast implants. Atlanta TV commercials tell us that “diamonds are forever,” right? Unfortunately, breast implants aren’t.
Why Replace Breast Implants?
There are several reasons you might wish to switch out your implants in a procedure called a breast implant revision. Here are a few of them:
1. A desire to change implant size (larger or smaller)
2. A desire for a change in shape or projection
3. A wish to switch from saline-filled to gel-filled or vice versa
4. A capsular contracture
5. Deflation of saline implants (one or both)
6. Visible wrinkling, rippling, bottoming out or the presence of the “double bubble”
7. Breast sagging that requires implant changes during a breast lift. It’s not always necessary to replace the implants in a breast lift performed long after breast implants were inserted. Much depends on the age of the implants and whether or not my patient is satisfied with the size of her breasts, not withstanding her dissatisfaction with the sagging of them.
How Long Do Implants Last?
While most women are told that their implants will last about ten years, some women will have no need or desire to change implants for 20 years. In other words, it varies among women.
Will There Be More Scars if I Replace My Breast Implants?
Usually no. Unless the scars are hypertrophic, the same incisions are used as were used in the initial breast augmentation procedure.
How Do I Know If My Breast Implants are “Too Old?”
Unless you have one of the problems noted in the list above, if it’s not broken, don’t fix it. If you have any concerns about your implants, however, set an appointment to see me. Remember, in saline-filled implants a rupture is obvious, but a gel-filled implant failure isn’t. If you’re experiencing “women’s intuition” about your implants, come in and I’ll examine you. If I think you need an MRI to identify a possible gel-filled implant failure, I’ll tell you. If we aren’t sure, you can decide if you want to have an MRI. But be seen and put your mind at rest about it.

It’s easy to forget that plastic surgeons
are “also” doctors—by that I mean I’m
not focused only on your appearance—I

What is Safe Beauty?
Safe Beauty is focused on knowing the
ingredients of cosmetics and skin treatments, and increasing your awareness
about how chemicals in makeup, fragrances, personal care products, and skincare
products might affect you both in the short- and the long-term. It’s an
education for me as well! As q surgeon, and an aesthetic surgeon to boot, I
want to be certain that the treatments I or my staff perform will improve your
appearance now, of course, but I also want to be certain that there aren’t any
untoward side effects later!
The
Fox is Guarding the Hen House
Did you know
that, even though the Food & Drug Administration monitors food and drugs,
the FDA neither has the power to oversee nor regulate the $50 billion personal
care products industry? Did you know that many
ingredients in soap, shampoo, body wash, baby care products, fragrances and
cosmetics are not on the labels? Would you ever
guess that personal care products labeled as “organic” can be sold without a
single organic ingredient in them? And that manufacturers can include the word
“natural” even if the product is filled with synthetic ingredients? It’s
completely legal. Further, companies can make all kinds
of claims, including false ones, about product effectiveness, because neither
the FDA nor anybody else regulates the language on the labels. Disheartening,
isn’t it?
Many of us are so overwhelmed
with the constant bad news about the chemicals sprayed on our food, leached
into our water, buried in the earth and floating in the air, that we want to
throw up our hands and cry, “Uncle.” But
in truth, cosmetics and home care products are one area in which you have a
great deal of control!
You can (and should) know what
you’re putting on your body, just as
you want to know what you’re putting in
it. Further, you can and should know how effective a product really is—based on scientific testing,
not just because the label says so!
Everything we do here at our Atlanta plastic surgery
practice has undergone rigorous testing and scrutiny to
assure our patients of not just beautiful results, but the highest level of safety as well. My medical and plastic
surgery training focused on perfecting tried-and-true procedures, to enable me
to safely create physical improvement for patients desiring cosmetic
enhancements. The equipment we use has been tested for safety and efficacy. The
injectables, such as BOTOX® Cosmetic, Juvéderm, Radiesse, and all the others,
have undergone years of testing before the FDA gave their federal stamp of
approval. The skincare products we provide our patients have undergone rigorous
testing as well.
Why Consumers Haven’t Been Protected
The existing law, the Food,
Drug & Cosmetics Act of 1938, as you can see by its date, is vastly behind
the times. That law gave decisions regarding ingredient safety to the cosmetic
industry—similar to allowing the fox to guard the henhouse, wouldn’t you agree?
Many of the ingredients in today’s personal care products didn’t even exist in 1938! Under the 1938 law, the
FDA cannot force cosmetics companies to verify the safety of the products they
sell, nor can it issue a product recall. When Brazilian
Blowout hair straightening products, used on children as young as 5 years
of age, were discovered to contain formaldehyde, the FDA was powerless to act.
A letter was issued, but in truth, the FDA cannot close them down, even though
the National Academy of Sciences released a report on formaldehyde that
confirms the determination of the Environmental Protection Agency that
"formaldehyde causes cancer in humans."
A New Law Is In The Works
The Safe Cosmetics Act of 2011, currently House Rule 2359, was
introduced in June of 2011 by Representative Janice Shakowski, and co-sponsored
by 20 other legislators. The Act would amend the 1938 act to allow the
Secretary of Health & Human Services to regulate the cosmetics industry in
the following ways:

To require that labels clearly identify all ingredients. Many ingredients in
products we use daily are never posted on the labels;
·
To establish safety standards to avoid consumer harm,
especially in products used by children, the elderly, and cosmetics industry workers;
·
To notify workers in cosmetics plants of any heath
risks associated with the products they manufacture and package;
· To eliminate ingredients linked to
cancer, birth defects and developmental harm;
·
To establish a database of cosmetic ingredients. This
database would include three ingredient categories: (A) a prohibited/restricted
list; (B) a safe list and (C) a priority assessment list for new ingredients
not yet classified;
·
To require manufacturers to report any adverse health
effects associated with their products;
·
To minimize the use of animal testing through shared
product databases;
·
To create an Interagency council on Cosmetic Safety;
·
To provide additional funding to the FDA Office of
Cosmetics and Colors so it has the resources necessary to oversee the cosmetics
industry.
·
To permit the FDA to order recalls for any cosmetics
in violation;
What
Can You Do as a Consumer?
Awareness is the first step. Yes, it’s
painful to discover that favorite lipsticks, mascaras and baby products are
tainted with chemicals like lead and formaldehyde, but hiding one’s head in the
sand won’t make the risks disappear. Knowledge
is power. What you learn over the next 12 months can help you lower your
exposure to chemicals linked to allergy; skin and scalp irritation; hair damage
and hair loss and worse, cancer, Alzheimer’s, and a host of other
chemical-related disorders.
